2. Getting someone admitted to a nursing home

If my family member is coming from a hospital, what should the hospital and/or I provide to have my loved one admitted to a MediLodge nursing home?

Doctor’s or hospital referral, which includes: Doctor’s orders, History and Physical, current chest X-ray, Mental Health Screening forms (MIMR)
Completed application
Insurance cards
DPOA papers/Patient Advocate, Guardianship/Conservatorship
5-7 days of clothes


If my family is coming from home, what do I need to do to bring my family member into a nursing home?

If the family member is coming from home, we will need specific paperwork completed. We will need to review medical information and let you know if we have an appropriate bed for your family member. Here is a list of needed information:

Current chest X-ray within a 30 day period
History and Physical with diagnosis (medical) and Mental Health Screening Forms (MIMR)
Information about normal, daily routines
Current list of medications and doses
Legal Documents (Power of Attorney or Guardianship)


What personal belongings can we bring to make our loved one feel more at home? (All items are optional.)

Medicare Card and other insurance card and Social Security Card
TV and remote control
Radio
Pictures and other personal items
Comforter
Pictures and/or favorite knick knacks
Small rocker or recliner as room space allows
